APPLICATION FOR SERVICE

You may view and print our application by clicking on (or tabbing to and pressing "enter") Application For Service as a Word Document or Application For Service in Adobe PDF Format. Since we are required to maintain a file of original certifying signatures, please print out the application, have it signed, and mail it to us. If you are applying to start service for an institution such as a school or nursing home, please have your Principal or Administrator provide the certifying signature on Page 2. You will receive a confirming phone call as soon as we receive your application.

HOURS OF PUBLIC SERVICE:

The Library is open for public service on weekdays, Monday through Friday, from 8:30 a.m. to 5:00 p.m., except for regularly scheduled holidays. Feel free to call ahead to verify open hours.

DIRECTIONS to the Washington Talking Book & Braille Library building:

The entrance to the Library is on the corner of 9th and Lenora -- 2021 9th Avenue, Seattle, Washington.

BUS: Most bus lines are within 5 blocks. Local services include routes 5, 8, 26, 28, 174, and 355. The Library is two blocks northwest of the Seattle Greyhound bus station.

SEATTLE STEETCAR: The Seattle Streetcar has a stop on the corner of 9th Avenue and Westlake Avenue North. The Library is one block southeast on 9th Avenue.

DRIVING: From north or south I-5, exit at Mercer. Turn left at the light onto Fairview. Stay in the right lane. Just beyond the Seattle Times building, cross Denny and Boren. Bear straight ahead onto Virginia. Go one block, turn right onto Terry, then left onto Lenora, then left onto 9th.

PARKING: Parking spots for vehicles with State Disabled Parking Permits are available at the curb immediately in front of the building on 9th Avenue or at the south end of the building up a short ramp off 9th Avenue into the parking garage. Stairs or an elevator provide access back down to the first floor entrance from the garage. Metered parking and pay lots also are available in the area. After 10 a.m., check the commuter parking spaces on 9th between Lenora and Virginia -- they are free.
